Post-operative pain remains a major problem following surgery for lesions of the skull base. Transcranial nerve stimulation (TNS) has recently been developed as an effective and safe treatment modality for chronic pain syndromes. We have assessed the effectiveness and safety of peri-operative TNS in the management of post-operative pain following skull base surgery. We retrospectively identified 11 patients who underwent surgery for benign lesions of the anterior cranial fossa between December 2007 and July 2009 and who received pre- and post-operative TNS. Patient characteristics, pain scores and side effects of TNS were evaluated. Eight patients received unilateral TNS with an electrode placed on the supraorbital nerve (SOn), two patients received bilateral TNS with a unilateral electrode placed on the greater occipital nerve (GOn) and one patient had an electrode placed on the supraorbital, superior trigeminal (V3) and facial (VII) nerves. In eight patients, TNS was performed with an external stimulator (PharmaStim, NeuroConn Inc, Germany). In two patients, TNS was performed with a NeuroTrace stimulator (Medtronic Neuro-stim, Texas, USA) to identify the affected nerves. Four patients had >30% pain reduction from baseline pain, while one patient had a >50% reduction in pain following surgery. One patient had a >50% increase in pain following surgery. One patient had a 50% reduction in pain at one week but experienced a relapse at two weeks post-operatively. No patients experienced TNS side effects.
